It is not necessary to read all of the consent forms listed below. Simply select the consent form that best describes your role as a potential participant in the study.
A primary driver is the person who owns the study vehicle and is the primary driver of that vehicle.
A secondary driver is someone who may also drive the study vehicle on a fairly regular basis such as a spouse, child, or friend.
Individuals under the age of 18 cannot legally grant consent, so we seek their assent to participate in the study.
Teen assent must also be accompanied by parental/guardian consent.
A parent/guardian must consent for a child under the age of 18 years to participate in the study
